Chest Pain Symptoms You Should Never Ignore

Chest pain linked to the heart often follows recognisable patterns. It is not always severe, but it is rarely random. Seek prompt assessment if you experience:
Pressure or tightness in the centre of the chest

Breathlessness during minimal activity

Cold sweats, nausea or lightheadedness

If something feels different from your usual state of health, do not ignore it. Even if symptoms settle, the underlying cause may persist.
Mild Does Not Mean Harmless
Heart-related chest pain is not always severe. It may be mild, intermittent or triggered by exertion. It may settle with rest.

The apparent intensity of symptoms does not reliably reflect the degree of underlying coronary disease. In some patients, the first clear manifestation of heart disease is an acute cardiac event.

Because chest discomfort can arise from both cardiac and non-cardiac causes, careful evaluation is required to determine what it represents.

If you are unsure whether your symptoms are significant, that uncertainty alone is a reason to seek assessment.

How Chest Pain Can Present?

Chest pain does not follow a single pattern. It may present as:
  • Sudden, sharp discomfort

  • A gradual build-up of pressure or tightness

  • Heaviness that feels like weight on the chest

  • Burning that resembles indigestion

  • Intermittent discomfort that comes and goes

  • Pain that appears with exertion and settles with rest

  • Symptoms that occur at rest or during sleep

  • Breathlessness without obvious chest pain

Episodes may be brief or prolonged. However, severity is not a reliable indicator of underlying risk.

When Should You See a Doctor?

Do not wait for pain to become severe before seeking clarity.
Arrange a medical review if:
  • Chest discomfort is new or unfamiliar

  • The pattern has changed in frequency or intensity

  • Symptoms last more than a few minutes

  • Discomfort recurs with exertion

  • Breathlessness, faintness or cold sweats accompany the pain

  • You have risk factors such as diabetes, high blood pressure, high cholesterol or a family history of heart disease

Uncertainty is not reassurance. Early assessment allows risk to be identified before an emergency develops.

Not all chest pain originates from the heart. However, the distinction cannot be made through assumption alone. Other possible causes include:


  • Acid reflux or oesophageal irritation
  • Muscular strain or inflammation of the chest wall
  • Lung conditions such as infection or inflammation
  • Anxiety-related episodes

Clarity comes from evaluation, not guesswork.


The priority is to exclude coronary artery disease first, before attributing symptoms to less serious explanations.

Dr Eric Hong is one of the few cardiologists in Singapore who is doubly accredited in both interventional cardiology and non-invasive cardiac imaging after completing two ACGME-accredited clinical fellowships in the United States. He was the first Singaporean accepted into the clinical programmes at Brigham & Women’s Hospital, Harvard Medical School, where he completed dual fellowships and contributed to research and development in advanced cardiovascular imaging and next-generation stent technologies.
